Landscape and Architectural Request Form

• All Homeowners are required to submit any proposed architectural / landscape changes / modifications to the exterior of your home or yard to the HOA Board for approval, prior  to commencement of any work. This includes improvements, additions, changes of any kind, replacements or relocations.

Follow these steps to successfully complete the request

• Review Bella Vista Rules and Regulations document, which is available for download below this form.  Ensure that your proposal complies with ALL guidelines pertaining to your project or it will be subject to rejection.
• Attach documentation describing, in detail, the changes, modifications or improvements. This must include:

1. Scope of the work
2. Proposed design
3. Descriptions, drawings, dimensions and location on property or home using plot plans/property boundaries.
4. Pictures, brochures or website addresses of materials, colors or fixtures being used
5. Copy of contractor(s) bids (prices may be removed or blackened out). Contractors must be insured and licensed.

• Obtain any applicable County / City / Township permits and submit copies to HOA upon obtaining.
• Agree to call PA One-Call, if work requires ANY digging to avoid hitting gas/power lines (dial 8-1-1)
• Agree to make repairs to any affected areas that have been disturbed by the contractors in any way.

Electronic Submission Agreement

• By my submission of the electronic form below and attachment of supporting documents, I (as the legal homeowner of the address provided) certify that all work will be completed with full compliance to all federal, state and local laws, building codes and ordinances.  
• I am responsible for locating all property lines, setback lines, easements, right-of-way, flood areas, etc.  The Association and its members are not held liable for any aspect of the work performed.  
• This request form is to obtain approval for the scope of work being completed.  Work may not commence, until fully approved by the Executive Board, which may take up to 14 days for approval.  
• I will adhere to and follow all Bella Vista Association Rules and Regulations as outlined in the attached LAR document, including timely completion.
